Passport & Photo Services

The Willowick and Willoughby library buildings of the Willoughby-Eastlake Public Library are official Passport Acceptance Facilities for the US Department of State.

Appointments

Passports are available by appointment only. Please call (440) 943-4151 (for Willowick) and (440) 942-3200 (for Willoughby) during the following times:

  • Monday – Thursday, 10 am – 8 pm
  • Friday – Saturday, 10 am – 4 pm

The application process can take 20-60 minutes per applicant. To ensure ample time, please arrive at least 60 minutes prior to passport hours closing. More time may be needed for complex cases involving citizenship, parental guardianship or last-minute travel. You may be asked to return at another time if the passport acceptance agent cannot complete your application prior to closing.

The US Department of State has the sole responsibility of examining applications and issuing passports.

Applying

If this is your first passport, you must apply in person. You will need:

  • A completed unsigned application for passport (Form DS-11 ) filled out in black ink
  • Proof of US citizenship
  • Current proof of identity
  • One passport photo
  • Form DS-82 U.S. Passport Renewal Application for Eligible Individuals by Mail

    If you were born in Ohio, you can visit the Department of Health’s Vital Records page to get a copy. If you were born in another state, visit the Centers for Disease Control website for a state-by-state listing of contact information for obtaining vital records.
Children under 16 years old

Children under age 16 require both parents or legal guardians to be present during the application process. Birth certificates must be submitted for all children showing parental relationship, even if the child has had a passport previously. Children ages 16 to 17 can get passports, but they must apply in person at an official passport acceptance facility. Renewing a Passport

If you are renewing a passport, be sure to read over the requirements. The applicant is responsible for mailing Renewal Applications to the U.S. State Department.

Online Passport Renewal (OPR) is now available to the American public. Applicants who meet the requirements can now renew their passports online at their convenience and from the comfort of their own home. Applicants can see a full list of requirements at travel.state.gov/renewonline. It’s still important for applicants to plan and apply for their passport well in advance of travel for both OPR and regular mail-in passport renewal service.

Fees

There are several different passport fees that may apply. Some fees are paid directly to the US Department of State and others are paid directly to Willoughby-Eastlake Public Library. For more information on fees, please visit travel.state.gov.

  • Execution fee: $35 for each new passport, payable by credit or debit card, cash, check, or money order made payable to Willoughby-Eastlake Public Library
  • Passport photo fee: $10 payable by credit or debit card, cash, check, or money order made payable to Willoughby-Eastlake Public Library

There are fees associated with the application process. Any fees being paid to the State Department must be paid via check or money order. For reference, the current fees are as follows:

Receiving your Passport

Updated Processing Times – Effective October 4, 2024

We are offering faster processing times to customers who apply on or after November 6:
Routine Service: 4 – 6 weeks
Expedited Service: 2 – 3 weeks

Our processing times only include the time your application is at one of our passport
agencies or centers and do not include mailing times, which may vary across the
country.
