Golden Buckeye Applicants
How to Qualify
Ohio residents who hold current Ohio Driver Licenses or State ID cards automatically receive a Golden Buckeye card in the month of their 60th birthday. Individuals who do not have a current license or State ID, as well as people under age 60 who are eligible due to disability, or who otherwise do not receive their card automatically, may apply for a card at the library.
Applicants must be at least 60 years of age. Applicants just turning sixty may apply the first day of the month in which they are going to be sixty. Applicants must be over the age of 18 and provide documentation from a state or federal agency that has the authority to certify individuals as disabled, or a statement from a licensed medical practitioner. The physician’s statements must say that the applicant is totally and permanently disabled and is unable to work at any substantially remunerative employment.
